Researchers led by Cleveland Clinic scientist Stanley Hazen tracked more than 3,000 adults in the United States and Europe. People with the highest blood xylitol levels faced a 1.57 times higher risk of major cardiovascular events like heart attack, stroke and death within three years, compared to those with the lowest levels.

The study didn’t stop at one data set. A separate Canadian cohort followed for six years found people in the highest xylitol group carried a 57% greater risk of major cardiovascular events, even after adjusting for smoking, diabetes, blood pressure and cholesterol. A third group followed for 30 years in England showed an 18% higher risk in the top group.

Why Sticky Blood Platelets Are The Real Concern

Numbers alone don’t explain why xylitol matters. The deeper worry is what it does inside blood vessels. Hazen’s team ran lab and animal tests showing xylitol made platelets more likely to clump together and form clots, the exact process behind most heart attacks and strokes.

An editorial published alongside the study called the three-year results “statistically significant and clinically relevant,” a rare stamp of seriousness from fellow scientists reviewing the same data. Later commentary in the same journal noted the platelet findings held up across lab dishes, animals and human blood samples tested outside the body, strengthening the case that this isn’t a fluke.

Regulators Still Call It Safe, But Doctors Are Splitting

The FDA still lists xylitol as safe for use in food and oral care products, and UCLA Health’s own writeup echoed that stance, telling readers moderation is the real caveat, not avoidance. That regulatory comfort is exactly why some doctors are frustrated by how slowly warnings have spread to consumers.

Cardiologists quoted by Cleveland Clinic weren’t shy about their concerns. Hazen himself said elevated xylitol levels “contribute to heightened platelet reactivity” and raise real risk for heart attacks and strokes, not a hypothetical one. Other physicians pushed back gently, arguing average consumers eating normal amounts of sugar-free candy likely face little danger, while heavy users of sweetener-loaded products deserve more caution.

The Bigger Picture For Sugar-Free Shoppers

Consumers have long been skeptical of food industry claims that engineered substitutes are automatically healthier than the real thing. This study gives that instinct some scientific backing. Xylitol was sold for decades as the safe, natural-sounding alternative to sugar, and now serious researchers are asking hard questions regulators haven’t caught up to yet.

This isn’t a reason to panic over one stick of gum. It is a reason to read labels, especially on products marketed as diabetic-friendly, keto, or “sugar-free,” and to push for the follow-up studies researchers themselves say are overdue before anyone declares xylitol safe again with full confidence.
